Anticoagulation Clinic

The Anticoagulation Clinic is a physician-directed, pharmacist-managed outpatient clinic for patients on warfarin therapy. Anticoagulation clinic patients require an order from their physician and then must call to make an appointment. This service is offered Tuesday through Thursday. A modified schedule can be provided based on patient needs and priorities.

Our Focus

To assist physicians in managing oral anticoagulation therapy (Warfarin) by evaluating the prothrombin (PT) and/or the International Normalized Ratio (INR) and instructing patients or family, of the appropriate dosage of Warfarin.

To assess patients for possible complications related to anticoagulation therapy.
To provide comprehensive and ongoing education to patients and/or family members about anticoagulant therapy with specific attention to signs and symptoms.

Tuesday– Thursday: 8:00 a.m. – 3:30 p.m.
