Anticoagulation Clinic

The Anticoagulation Clinic is a physician-directed, pharmacist-managed outpatient clinic for patients on warfarin therapy. Anticoagulation clinic patients require an order from their provider and then must call to make an appointment. This service is offered Tuesday through Thursday. A modified schedule can be provided based on patient needs and priorities.

Our Focus

To assist providers in managing oral anticoagulation therapy (warfarin) by evaluating the prothrombin (PT) and/or the International Normalized Ratio (INR) and instructing patients or family, of the appropriate dosage of warfarin.

To provide comprehensive and ongoing education to patients and/or family members about anticoagulant therapy with specific attention to signs and symptoms with specific attention to signs, symptoms, and possible complications.
